There is an NDIS Access form that will need to be completed by yourself and treating health professional. Once completed and evidence is submitted that you have a lifelong disability, under the age of 65 and being an Australian Resident you may be eligible for NDIS supports

There are 3 categories Core, Capacity and Capital funding. Core is for personal care, transport and Community access, Capacity Building is working with your choices of how you would like to manage the funding and Allied Health Professionals. Capital if for High Cost Assistive technology, Specialist Disability Accomodation (SDA) and home and Vehicle modifications

If there have been significant changes in your life and your capacity you may be able to request a change of details, circumstances for the NDIS to determine what is reasonable and necessary
